Sure enough, B12 had dropped from 620 to 340 pg/mL. Started B12 injections monthly and MCV normalized within 2 months. GLP-1 agonists can impair B12 absorption — this is a real clinical concern that deserves routine monitoring.
Adding the numbers, since they settle part of this. The boring version of this is the one that works, and the boring version is: measure a baseline, change one variable, wait, measure again under the same conditions. Nobody wants that answer and it is still the answer.

Coming at Dr.SurgeonPGH’s question from a different direction. Take it one variable at a time. Almost every unanswerable question in these threads is unanswerable because three things changed in the same fortnight, and no amount of subsequent argument can untangle them after the fact.

Following up because I said I would. I changed one thing, left everything else alone, and waited the full interval before looking. It resolved, and I cannot prove which half of that did it.
