We get questions about peptide stacking every day, so I'm creating this reference thread. The most common stack our members use is GLP-1 RA + BPC-157 + TB-500. Let's discuss the safety considerations systematically.
The fundamental question: are there dangerous interactions between these peptides?
Short answer: no known dangerous pharmacological interactions. But "no known interactions" is different from "proven safe in combination," and I want to be transparent about what we know and don't know.
What we know:
1. GLP-1 RAs (semaglutide, tirzepatide) work through incretin receptors — GLP-1R and GIP-R. These are G-protein coupled receptors primarily in the pancreas, gut, and CNS.
2. BPC-157 works through multiple pathways: nitric oxide system, FAK-paxillin signaling, VEGF upregulation, and dopamine/serotonin modulation. It does NOT interact with incretin receptors.
3. TB-500 works through intracellular actin regulation, promoting cell migration and angiogenesis. It does NOT interact with incretin or nitric oxide pathways significantly.
These three peptides act through distinct, non-overlapping receptor systems. There's no known receptor competition, metabolic interference, or pharmacokinetic interaction between them.
What we DON'T know:
1. No formal drug interaction studies have been conducted between any of these compounds
2. Downstream signaling convergence is possible but unstudied — for example, both GLP-1 and BPC-157 affect inflammatory pathways, so additive anti-inflammatory effects are possible (probably beneficial but unmeasured)
3. Long-term safety of any combination beyond anecdotal reports does not exist
4. Individual genetic variation in metabolism could create unexpected interactions

Been running GLP-1 + BPC-157 + TB-500 for 14 weeks. Here's my detailed protocol and experience:
Stack:
- Tirzepatide 7.5mg weekly (prescribed, for weight management)
- BPC-157 250mcg subQ 2x daily (for GI side effect management + shoulder tendinopathy)
- TB-500 2.5mg subQ 1x weekly (for shoulder tendinopathy)
Timing:
- Tirzepatide: Sunday evening
- TB-500: Wednesday evening (separated from tirz by 3 days)
- BPC-157: Morning and evening daily, rotated injection sites (abdomen and near shoulder)
Labs at baseline vs week 12:
- CBC: No significant changes
- CMP: All within normal limits both times
- Liver enzymes: ALT went from 28 to 22 (improvement, likely from weight loss)
- Lipids: Total cholesterol dropped 35 points (likely tirz + weight loss)
- CRP: 4.1 → 1.3 (significant improvement)
- No new abnormalities
Adverse events: None attributable to the stack. The GI side effects from tirzepatide were significantly reduced with BPC-157 (as discussed in other threads). No injection site reactions beyond occasional mild redness.
Subjective results:
- Weight loss: 22 lbs in 14 weeks
- Shoulder: About 70% improvement in pain and ROM
- Energy: Good
- Sleep: Improved (though hard to attribute to any single factor)
The biggest practical concern with stacking isn't pharmacological — it's injection fatigue. Between weekly tirz, weekly TB-500, and twice-daily BPC-157, I'm doing 16 injections per week. This gets old. I've considered switching BPC-157 to the oral stable form to reduce the injection burden.

The injection burden point is important and under-discussed. Here's how I optimized my stack to minimize injections:
Consolidation strategy:
- BPC-157 and TB-500 can be mixed in the same syringe. They're chemically compatible in solution. Draw up BPC first, then TB-500 into the same syringe.
- This cuts your non-GLP-1 injections from 15/week to 8/week (2 daily mixed BPC/TB + 1 weekly TB-only for the higher dose)
Actually, let me recalculate. If you do:
- AM: BPC-157 250mcg (daily) — 7 injections/week
- PM: BPC-157 250mcg + TB-500 (on TB day, just add to the syringe) — 7 injections but one has TB mixed in
- Tirz: 1 injection/week
That's 15 injections. To reduce further:
- Switch BPC-157 to once daily at 500mcg: 7 injections/week total (including tirz and TB)
- Or switch BPC to oral stable form: 2 injections/week (tirz + TB)
Most people who run long-term stacks eventually settle on the minimal injection protocol to maintain compliance. A protocol you actually follow consistently beats a "perfect" protocol you abandon after 6 weeks.

I just started semaglutide 0.25mg two weeks ago. How long should I wait before adding BPC-157 and/or TB-500?
My reasoning for wanting to add them:
- BPC-157: preventative GI support as I titrate up
- TB-500: chronic Achilles tendinopathy that limits my exercise
Is it better to add one at a time to isolate effects, or can I add both simultaneously?

Good instinct to ask about timing. My recommendations:
Wait at least 4 weeks on semaglutide alone before adding anything. You need to:
1. Establish your baseline response to semaglutide
2. Identify which side effects are from the GLP-1 vs other variables
3. Complete at least one dose escalation to understand your tolerance
Then add ONE peptide at a time, with at least 2 weeks between additions. This way, if you experience any new symptom, you can attribute it. If you add BPC-157 and TB-500 simultaneously and develop, say, a headache — you won't know which one caused it.
Suggested timeline:
- Weeks 1-4: Semaglutide alone (0.25mg → 0.5mg)
- Week 5: Add BPC-157 at 250mcg once daily
- Week 7: If tolerating well, increase BPC to 250mcg twice daily
- Week 8: Add TB-500 at 2.5mg once weekly
- Ongoing: Adjust doses based on response
For your specific goals, I'd actually add BPC-157 first since it may help with GI tolerance as you titrate semaglutide, AND there's evidence it promotes tendon healing which could help your Achilles alongside the TB-500 you add later.
